Requirements for you to become an Accredited NESTA Personal Fitness Trainer:

– Must have paid in full prior to examination date
– Must be at least 18 years of age
– Must have a valid Driver’s License
– Must have a minimum high school level or equivalent education
– Must have current Emergency Cardiac Care (CPR) Certification card/certificate or BLS (Basic Life Support) card/certificate in place of CPR card
– Must to maintain the confidentiality of the exam content and abide by exam policies as stated in the candidate handbook
– Must agree to adhere to the NESTA Code of Ethics
– Exam candidate may be subject to an audit to verify proof of documentation requirements (show proof of valid CPR card/certification, Driver’s License or Passport, High School Diploma or equivalent)
– Must complete the 125 question, multiple choice, facilitated proctored exam within 90 days of receiving exam eligibility ID
– PFT exam candidates must take the NESTA proctored exam at a PSI testing center

Personal Fitness Trainer Exam Details

  • 125 question, 2-hour proctored multiple choice computer web based exam
  • The exam candidate must take the Personal Fitness Trainer proctored exam within 90 days upon receiving their exam eligibility ID.
  • One exam attempt per purchase (unless otherwise specified under purchase options).
  • Exam results are shown immediately upon completing the exam.  All passing exams, receive a temporary certificate at the time of completion.

Exam Locations

NESTA Personal Fitness Trainer exam candidates will take the exam at a PSI testing center in their local area. PSI is the proctored exam provider with one of hundreds of testing sites nationally throughout Canada and all 50 states in the U.S. including, Washington DC, Guam, Virgin Islands and Puerto Rico.

Off-Site Proctoring Exam Option for International Candidates and Scheduling Hardship 
International and scheduling hardship exam candidates will be required to use “Off-Site Proctoring Exam Option” and will not be eligible to test at a PSI facility.  Please visit Off-Site Proctoring Exam Option page for detailed information and instructions. Approval from NESTA is required prior to testing.

Individuals with Disabilities and Special Accommodation Needs 
Individuals having a disability requiring special accommodations must fill out the “Request for Special Accommodations” form. Requests are decided upon an individual basis. Accommodations usually include an allowance for increased examination time. Accommodation requests will be acknowledged through email and/or phone notification within 7-10 business days of initial request.

For individuals who require a reader, separate accommodations or other requirements please state your needs and requirements in detail on the Request for Special Accommodations Form. Requests will be considered and a response will be delivered within 10 business days. Not all PSI facilities are equipped to provide special accommodations requests.

Recertification and Continuing Education Process

The NESTA ECB requires its certified personal fitness trainer (PFT) to complete 4.0 Continuing Education Units (CEUs) over a four year period. Each 0.1 CEU is worth one hour of study time or one contact hour in a live training format.

Some owners may simply want their dog to appear more muscular. The motivation behind this may be to give a guard or watch dog a more intimidating appearance as they help protect property and their people.

Others may want their pet to better fit the description of a show dog to improve their chances of placement and make them more competitively successful. Owners of athletic dogs on a special diet may also give these supplements to optimize muscle building.

2. Build strength

Owners who routinely enter their dog into competitions that have multiple categories not just based on appearance, may choose to give their canine muscle supplements in order to make them stronger. There’s a number of dog sports where such supplements may be advantageous for the animal.

Whole Body Vibration Shakes Up Microbiome and Reduces Inflammation in Diabetes

Prior studies in mice have shown that whole body vibration (WBV) can mimic some of the positive effects on health of exercise, and even reverse some of the symptoms of type 2 diabetes. New research by a team at the Medical College of Georgia (MCG) and Dental College of Georgia (DCG) at Augusta University has provided new clues as to the mechanisms involved. Their studies in a mouse model of obesity showed that WBV results in increased levels of inflammation-suppressing immune system macrophages, and high numbers of gut bacteria that makes short-chain fatty acids (SCFAs), which can help the body better utilize glucose.

The findings “… support the notion that WBV has the potential to alter the microbiota in a way that triggers innate and mucosal immunity to produce anti-inflammatory responses, down-regulating the hyper-inflammatory state and reversing the adverse consequences,” the investigators wrote in their published paper in the International Journal of Molecular Sciences. “More studies are required to solidify this novel approach, which can be a very affordable and effective therapeutic modality in the prevention and treatment of many diseases, including diabetes and obesity.” The researchers, headed by Jack Yu, MD, chief of pediatric plastic surgery at MCG, and Babak Baban, PhD, immunologist and intern associate dean for research at DCG, reported their findings in a paper titled, “Whole Body Vibration-Induced Omental Macrophage Polarization and Fecal Microbiome Modification in a Murine Model.”

The combination of high-fat, sugar-heavy diets and “massively reduced physical activities” is largely responsible for what the researchers called “an epidemic of obesity and chronic metabolic diseases,” including type 2 diabetes. Chronic inflammation is a major contributory factor to the development of metabolic and cardiovascular diseases, and the immune system’s macrophages play a key role in regulating inflammatory responses.

The potential to use WBV alongside more conventional measures for improving musculoskeletal health, and also potentially to impact on metabolic diseases such as diabetes and hypertension is gaining scientific interest, the authors continued. “Despite a certain level of controversy, all studies generally support the notion that WBV may improve the inflammatory indices and help to re-establish the immune balance and homeostasis.” However, they acknowledged, the mechanisms involved aren’t clear. “While the biochemical and physiological improvements from WBV as an exercise mimetic are indisputable, how WBV achieves such effects is likely multi-faceted and largely unknown.”

There are two types of macrophage. Those classified as M1 promote inflammation, while M2 macrophages suppress inflammation and play a key role in regulating inflammatory responses. Complex regulatory systems are involved in control glucose utilization, and research has implicated macrophages in feedback loops that foster inflammation and exacerbate insulin resistance and an inability to control glucose levels. The inflammatory status of macrophages also influences the gut microbiome, and the gut microbiome can impact on inflammatory responses, so the immune system and gut microbiome have what the investigators call “an intimate and reciprocal relationship, in that microbiota affects innate immunity and vice versa.”

Back in 2012, Yu reported at the Third World Congress of Plastic Surgeons of Chinese Descent that whole body vibration could reduce some of the effects of diabetes, such as excess urine production and excessive thirst. Those studies were in a mouse model that mimicked overeating adolescents. WBV also reduced inflammation levels, including shifts in some immune cell counts, and was more effective than drugs at reducing A1C (glycated haemoglobin), which is used as a measure of blood sugar levels.

For their newly reported studies, the scientists designed a series of tests in a leptin-deficient mouse model of diabetes to investigate whether WBV led to changes in macrophage type in blood and fat tissue, and/or changes to the gut microbiome. The results showed that WBV increased the numbers of M2 macrophages in both the diabetic and control mice, and restored M2 levels to those of normal controls. These results were “… consistent with our hypothesis that WBV induces macrophage polarization to M2 type in a diabetic mouse model,” the team wrote. WBV was also associated with increased levels of anti-inflammatory molecules including the cytokine IL-10, which were increased in both the diabetic and normal mice after vibration.

A program of six weeks of WBV (20 minutes per day, five days a week) was linked with changes to microbial composition and diversity. Overall microbial diversity decreased, but there was a major increase in Alistipes bacteria, which are normally only found in very small numbers in the microbiomes of healthy laboratory mice. Alistipes are non-alcoholic fermenters that produce SCFAs such as butyrate and acetoacetate, which are “known to be fuel for the gut flora, highly anti-inflammatory, and capable of reversing adverse effects of a high-fat diet.”

The team noted that while it is greater, rather than reduced microbiobial diversity that is thought to be beneficial to health, the reduced microbial diversity in WBV-treated mice may have resulted from an increase in beneficial species of bacteria that produce SCFAs rather than ethanol. Interestingly, Alistipes bacteria have been found in the gut microbiomes of hibernating mammals that undergo extensive nutritional and microbiological adaptations during the winter months.

The researchers claim that their study is the first to document crosstalk between the microbiome and innate immunity by altering the macrophage mix. What isn’t yet known is whether it’s the microbiome changes or the shift in macrophage type that comes first. “The sequencing of the WBV effects is still unclear,” they wrote. “Is M2 polarization antecedent to intestinal microbiome changes or vice versa? One of the next steps that may help to answer that question may be to carry out WBV tests in models in which macrophages are deleted.

“The sequencing is not yet completely clear,” Yu said, “but it appears to be a closed loop, feed forward, self-magnifying cycle.”

A Personal Trainer Gained 70 lb to Lose Weight With His Client, and It Did Work

Losing weight is not an easy journey, since you have to work out hard, cut unhealthy (but so delicious) food out of your life, and stay motivated at the same time. This story started with the TV Show Fit To Fat To Fit , where personal trainers aim to better understand their clients’ struggles by gaining weight and challenging themselves. When Adonis Hill was asked if he would gain 70 lbs for someone else, his answer was firm, “ If it  means saving their life, heavens yes !”
His client Alissa had been overweight her entire life. When she began this project, her weight was 308 lbs. Adonis had to gain more than 70 lbs in 4 months to match his partner. To do so he ate tons of junk food, drank soda, and stopped exercising.
His dedication caused some health issues. Doctors had to stop his weight gaining process one month earlier than they had planned because Adonis was diagnosed with hypertension and prediabetes. The good news is that he is no longer on meds since he returned his weight to normal. But he has no regrets and compares the risk he took with “a  fireman going into a burning building and rescuing someone trapped by a fire.”
When the first goal was hit, the second part of the journey started. Alissa and her selfless trainer worked together to get the bodies of their dreams. They ran, lifted weights, followed a strict diet, and supported each other through those difficult times. It was a real challenge since they had to fight not only lazy behavior, but also old addictions to unhealthy food and judgements from others, that have the power to ruin any aspirations.
After several months both of them achieved brilliant results. Alissa managed to lose 57 lbs in 4 months while Adonis reported that his weight was around 227 lbs. This means that this venture was worth the effort.
From his own experience, Adonis Hill showed that people can achieve anything if they are motivated enough. He  concludes that it was all about empathy, and the ability to understand and share the feelings of another person. With this experiment, the trainer hopes to raise awareness around obesity risks. “If we continued down that unhealthy road , the cemetery is where we would’ve ultimately ended up.”

In the face of diabetes, a common condition in which glucose and levels of destructive inflammation soar, whole body vibration appears to improve how well our body uses glucose as an energy source and adjust our microbiome and immune cells to deter inflammation, investigators report.

For the first time they have described how regular use of whole body vibration can create this healthier mix by yielding a greater percentage of macrophages — cells that can both promote or prevent inflammation — that suppress rather than promote.

In their mouse model, investigators at the Medical College of Georgia and Dental College of Georgia at Augusta University have also shown that whole body vibration alters the microbiome, a collection of microorganisms in and on our body, which help protect us from invaders and, in the gut, help us digest food.

Changes they saw included increasing levels of a bacterium that makes short chain fatty acids, which can help the body better utilize glucose, they report in the International Journal of Molecular Sciences. Glucose is used by the body for fuel but at high levels promotes inflammation, insulin insensitivity and ultimately can cause diabetes.

While there were other changes, the most dramatic they documented was the 17-fold increase in this bacterium called Alistipes, a gut bacterium not typically in high supply there but known to be proficient at making short chain fatty acids which, in turn, are “very good” at decreasing inflammation in the gut, says Dr. Jack Yu, chief of pediatric plastic surgery at MCG. Alistipes, which helps ferment our food without producing alcohol, generally improves the metabolic status of our gut and makes us more proficient at using the glucose we consume for energy.

When they saw this, co-corresponding authors Yu and Dr. Babak Baban, immunologist and interim associate dean for research at DCG, immediately thought that giving a dose of the bacterium, like you would a medication, with a smaller dose of whole body vibration — in this case 10 minutes versus 20 minutes five times weekly — might work just as well, and it did, they report.

It what appears to be this good chain reaction, when Alistipes went up, glucose use and the macrophage mix also improved, Baban says. “The sequencing is not yet completely clear,” Yu says, “But it appears to be a closed loop, feed forward, self-magnifying cycle.”

Our microbiome, like a casserole, is in layers and one way whole body vibration may work is by rearranging those layers, Baban says, but they reiterate that no one is certain just how whole body vibration works in this or other scenarios, like as an exercise mimic without all the proactive movement.

But it appears to help address a key concern in diabetes and many common diseases: inflammation. While acute inflammation helps us fight disease, chronic inflammation helps start and sustain a variety of diseases from cardiovascular problems to cancer as well as diabetes.

With rates of inflammation-producing obesity and related type 2 diabetes increasing — even in children — new therapies that can directly help avoid the health consequences are needed, they write. They add that while more work is needed, whole body vibration might be one widely applicable and generally safe approach to use.

Macrophages, which promote inflammation, called M1, and suppress inflammation, called M2, play an important role in regulation of the inflammatory response. The inflammatory status of macrophages also influences the gut microbiome and vice versa.

In Baban and Yu’s mouse model of type 2 diabetes, where circulating glucose levels are high, they wanted to know how whole-body vibration affected the inflammatory status of macrophages and the diversity of the microbiome. They theorized their diabetes model would have more M1s, and that whole body vibration would result in more M2s and yield changes in the microbiome as well.

They found both a significant increase in the number of M2s as well as increases in levels of other anti-inflammatory molecules like the cytokine IL-10, in both normal mice and their diabetes mouse model after vibration. In fact, whole body vibration restored M2 levels to that of normal controls.

In the microbiome, they saw numerous shifts but by far the most significant was the increase in Alistipes and a general decrease in the diversity of the microbiome.

They note that while more diversity is generally considered a good thing, in this case the shift likely resulted from an increase in species like Alistipes, which can produce short chain fatty acids like butyrate, which result from the fermentation of dietary fiber in our gut and which feed inhabitants of the microbiome, are highly anti-inflammatory and can help reverse ill effects of high-fat diets, they write.

Theirs is the first study to document crosstalk between the microbiome and innate immunity by altering the macrophage mix with whole body vibration. Innate immunity is a sort of basic defense that immediately responds to invaders in the body and includes physical barriers like the skin as well as immune cells like macrophages, which are key to this response. In this scenario, macrophages, for example, release other cells called cytokines that help trigger inflammation. Adaptive immunity is when the body makes a specific cell, like an antibody, to target a specific invader.

They note that it’s still impossible to know whether the microbiome or macrophage shift comes first but theorize that making more glucose available to macrophages fosters inflammation and insulin resistance.

Another experiment they want to do to better define the order, is to delete the macrophages and see if they still see the other effects, Baban says.

But either way, the investigators say the clear interaction provides more evidence that whole body vibration can turn down inflammation.

The microbiome lives in the mouth, gut, vagina and skin — mostly in the gut— at points where our body comes in contact with foreign items to help protect us from invaders. In the gut it helps us digest and use our food.

Scientists have found more than 8 million genes represented in the bacteria, fungi and viruses that comprise a healthy human microbiome while the human himself has more like 20,000 to 25,000 genes. Obesity has been associated with a less-diverse microbiome, which is actually more efficient at digesting food.

In diabetes, whole body vibration is known to reduce ill effects like excessive urine production and excessive thirst, Yu reported in 2012 to the Third World Congress of Plastic Surgeons of Chinese Descent. That work was in a mouse model, which mimicked overeating adolescents. Vibration also reduced inflammation levels, including shifts in some immune cell levels. Vibration also was better than drugs at reducing A1C levels, which provide a better idea of your average blood sugar levels than a fasting glucose by showing what percentage of your oxygen-carrying hemoglobin is routinely coated with sugar. High glucose, or blood sugar levels, may result in sugar binding to cells and other places inside the body where it can alter function.

“Hyperglycemia is not good,” says Yu. “When it happens you perturb the normal.”

While Alistipes, which does not survive well outside the body, is not currently a part of probiotics or even yogurt cultures, for these studies the investigators used levels of other bacterium, like lactobacillus, found in yogurt to determine how much to give when they tried the Alistipes as a medicine adjunct to whole body vibration.

Alistipes is found in plants, and levels have been shown to be decreased in individuals with inflammatory bowel disease and Crohn’s. Higher levels have been associated with depression, and high levels can be found in the gut of hibernating bears.

A 2017 study published in Endocrinology by Drs. Alexis Stranahan and Meghan E. McGee-Lawrence at MCG provided evidence that in their animal model of obesity and diabetes, whole body vibration was essentially the same as walking on a treadmill at reducing body fat and improving muscle and bone tone, including reducing seriously unhealthy fat around the liver, where it produces damage similar to excessive drinking.

Whole Body Vibration-Induced Omental Macrophage Polarization and Fecal Microbiome Modification in a Murine Model

Human nutrient metabolism, developed millions of years ago, is anachronistic. Adaptive features that offered survival advantages are now great liabilities. The current dietary pattern, coupled with massively reduced physical activities, causes an epidemic of obesity and chronic metabolic diseases, such as type 2 diabetes mellitus. Chronic inflammation is a major contributing factor to the initiation and progression of most metabolic and cardiovascular diseases. Among all components of an innate immune system, due to their dual roles as phagocytic as well as antigen-presenting cells, macrophages play an important role in the regulation of inflammatory responses, affecting the body’s microenvironment and homeostasis. Earlier studies have established the beneficial, anti-inflammatory effects of whole body vibration (WBV) as a partial exercise mimetic, including reversing the effects of glucose intolerance and hepatic steatosis. Here for the first time, we describe potential mechanisms by which WBV may improve metabolic status and ameliorate the adverse consequences through macrophage polarization and altering the fecal microbiome.
